The Care Manager will consult with the Resident, their family and carer to prepare a written plan of how the Resident’s needs in respect of their health and welfare are to be met. The plan will be reviewed and revised according to the changing needs of the Resident. The plan will be available to the Resident and any changes made will be discussed with them and their representatives.

A plan for a Resident’s care is essential to make sure they have all the support they need for their health and happiness in the home. This will empower the Resident to document their thoughts, needs and wishes, helping the staff to get to know them and deliver person-centred care.
